Abstract
Background and aim In general during pregnancy, women are aware of the importance of a good diet quality, are interested in nutrition and receptive to changing dietary intake. However, adherence to dietary guidelines is sub-optimal. A pregnant woman’s first information source regarding nutrition information is their midwife – making healthy nutrition promotion by midwives therefore very promising. The aim of this research is to develop an intervention, to be used by midwives and dieticians, to empower pregnant women towards improved diet quality.
Methods To develop an intervention in collaboration with stakeholders, six literature studies were combined with quantitative and qualitative studies, resulting in an iterative process. Quantitative questionnaires were administered among pregnant women (n=52) and dieticians (n=124). Interviews were held with pregnant women (n=27) and dieticians (n=13). Three focus groups were held with midwives (n=3, n=5, n=4), two with dieticians (n=5, n=12) and two with pregnant women (n=3, n=3). Three pressure cooker sessions were organized with various stakeholders, offline (n=15) and online (n=8, n=7). Individual and group discussions with several professionals (n=12) and an expert group (n=3) were regularly held to continuously refine the intervention.
Results The intervention Power 4 a Healthy Pregnancy [P4HP] consists of four consultations to specifically discuss the pregnant woman’s diet and seek for opportunities to empower towards improved diet quality. During the first consultation, the midwife identifies perspectives and discusses motivation of the pregnant women for a healthier diet quality. During the second consultation, the dietitian follows-up with a reflection and helps targeting the selected dietary changes to the woman’s food habits and specific situation. In the third and fourth consultation the midwife invites the woman to reflect on her dietary intake and discusses further improvements. P4HP can be delivered in an individual or group setting, is designed to be flexible and adaptable and uses a conversation tool and motivational interviewing.
Conclusion P4HP is designed to empower pregnant women towards improved diet quality. Further research will be conducted to implement and evaluate P4HP using a cluster randomized controlled trial to test our hypothesis that P4HP is beneficial for empowerment and diet quality (NTR Trial NL9551).
